Behavioral Health Social Worker

Triage, Assessment & Brief Intervention:

• Serve as primary intake clinician for escalated behavioral health and social needs consults.
• Conduct time-sensitive psychosocial and safety/risk assessments via phone, video, or ThymeBox.
• Deliver brief interventions for stabilization, including de-escalation and suicide risk management.

Escalation & Clinical Decision-Making:

• Use clinical judgment to determine escalation pathways for crises such as psychosis or suicidal ideation.
• Initiate escalations via required channels and notify clinical leads.

Care Coordination & Navigation:

• Provide high-complexity behavioral health management and coordinate with nurses, providers, and community organizations.
• Track referral status and ensure continuity of care until stable plan is established.

Consultation, Documentation & Team Support:

• Maintain timely documentation of triage assessments, safety plans, and referrals.
• Provide clinical consultation and decision support to care team members.
