The Role:
- Conduct initial assessments for patients enrolling in the virtual cardiac recovery program.
- Educate patients on program benefits and coach individuals through electronic messaging and video sessions.
- Complete individualized treatment plans including nutrition, psychosocial, and fitness assessments.
Your Experience:
- Minimum three years helping people manage their health in clinical or remote coaching settings.
- Degree in health-related field with counseling or education experience in disease management.
- Fluent in English and Spanish required.
