Responsibilities:

  • Serve as the first point of contact for members, handling calls, texts, and messages with empathy.
  • Schedule and confirm enrollment, follow-up, and wraparound visits.
  • Build ongoing relationships with members through proactive outreach and check-ins.

Qualifications:

  • High School diploma or equivalent and 1-2 years of patient care coordination experience.
  • At least 1 year of consistent EHR/EMR experience in scheduling or insurance verification.
  • Bilingual in English and Spanish and authorized to work in the US.
