Supervisor, Transitions of Care Coordinator

Accountabilities:

• Lead and support a virtual team of Transitions of Care Coordinators with day-to-day guidance and coaching.
• Dedicate about 30% of your time to direct patient support during transitions from hospital to home.
• Monitor team performance, attendance, and develop job aids to improve workflows.

Requirements:

• Medical Assistant certification desired, or significant MA experience with relevant documentation.
• At least 2 years of direct leadership or supervisory experience in a virtual environment.
• Strong technology skills and ability to adapt to evolving digital platforms.
