Strategic Leadership:
- Develop and implement comprehensive plans to drive patient throughput, efficient discharge planning, and readmission prevention.
- Drive continuous improvement in care management metrics, utilizing data analytics to enhance operations and patient outcomes.
Operational Oversight:
- Provide clinical direction for patient flow and ensure consistent application of systemwide standards and best practices for care coordination.
- Ensure adherence to all regulatory requirements and uphold the organization's mission and values.
Regional Expertise:
- Leverage deep understanding of local regulations and resources, building relationships with state agencies and community partners.
- Work closely with hospital and system leaders, as well as external stakeholders (payers, community resources), to achieve organizational objectives and reduce denials.
Team Development:
- Foster a culture of excellence, provide staff development, and ensure appropriate staffing models.
