Region Director Care Coordination

Strategic Leadership:

• Develop and implement comprehensive plans to drive patient throughput, efficient discharge planning, and readmission prevention.
• Drive continuous improvement in care management metrics, utilizing data analytics to enhance operations and patient outcomes.

Operational Oversight:

• Provide clinical direction for patient flow and ensure consistent application of systemwide standards and best practices for care coordination.
• Ensure adherence to all regulatory requirements and uphold the organization's mission and values.

Regional Expertise:

• Leverage deep understanding of local regulations and resources, building relationships with state agencies and community partners.
• Work closely with hospital and system leaders, as well as external stakeholders (payers, community resources), to achieve organizational objectives and reduce denials.

Team Development:

• Foster a culture of excellence, provide staff development, and ensure appropriate staffing models.
