The Clinical Services Consultant is a highly skilled RN leader responsible for providing interim clinical leadership across agency locations. This role involves supporting or temporarily managing all or part of the Director of Clinical Services/Area Director role remotely and on-site, ensuring high-quality patient care, regulatory compliance, and operational excellence. Up to 75% travel is required for this position.
This role plays a key part in our remote physiological monitoring (RPM) program by contributing to the development and oversight of individualized patient care plans and providing ongoing guidance and support to our patients. Monitor and review patient vitals and care plans using our proprietary technology platform and collaborate with registered nurse (RN) case managers to support the development and execution of care plans addressing clinical and social needs.
The Behavioral Health Medical Assistant provides administrative and clinical support to Behavioral Health Medical Provider staff and patients and works collaboratively with the multidisciplinary team to meet the needs of our Community Health Center patients. They assist in coordinating patient care, ensure patient-specific needs are met, and prepare patients for virtual appointments. They also verify and update patient information in the EHR and provide support to BHMP staff.
Provides high quality, accurate and compassionate care to a diverse patient population. An Ophthalmic Technician displays high proficiency in taking patient histories and is highly organized to fully prepare the patient for the physician appointment.
