Conduct a thorough evaluation of international organizations either on-site or remotely using systems analysis skills to determine compliance with standards. Engages with health care staff to identify opportunities for improving compliance and prepares exit reports.

As the Registered Nurse (RN) Team Manager, you will work closely with our providers and patients through many stages of their Thirty Madison journey, supervising a team that provides direct patient care and clinical support to KMG providers in delivering care to Thirty Madison patients. You’ll partner with and support our medical network and patients across all of our brands. This role will lead a team of RNs and Care Coordinators, optimize workflows, and manage performance metrics.

The Case Manager LVN/LPN will coordinate patient care over the phone between providers and caregivers to help ensure cost-effective, high-quality healthcare for health insurance plan participants. Manage cases on a short- and long-term basis over the phone and educate patients on treatment and services options.

Deliver coordinated, patient-centered virtual Care Management by telephone and video that improves members' health outcomes. Create impactful care plans together with members and our diverse care team, and help members achieve the desired goals. Help members navigate complex medical conditions, treatment pathways, benefits, and the healthcare system in general.
