Make Psychiatric assessments and evaluations of clients in an ongoing telemedicine capacity. Prescribe medication in an effort to reduce mental health symptoms, as needed. Create and collaborate with clients on care plans. Complete and/or administer appropriate paperwork and assessment tools for clients. Adhere to Thriveworks' Clinical Practice Guidelines. Consult with Regional Clinic Directors and staff on clients, programs and services at the various sites.

Deliver high-quality asynchronous primary care, manage a fast-paced caseload of at least 20 asynchronous cases per day, and build strong patient relationships. You will also master tech platforms, identify improvements, and communicate effectively, thriving in a fast-changing environment. Champion async-first care delivery, redefining how primary care is delivered efficiently and accessibly.

Reporting to the Regional Medical Leader, providers function as integral members of the Eleanor Health treatment team providing direct patient care for MAT, substance use and other psychiatric disorders both in-person and via telemedicine. Works closely with Eleanor Health Practice Administrator, Lead Clinician, and other team members to ensure seamless integration of medical services with whole-person support services.

This role involves seeing acute/urgent care patients in a live text-based chat clinic, including straightforward chronic care cases requiring refills, with 90% clinical and 10% administrative tasks, such as clinical meetings and EHR feedback. The Nurse Practitioner will accurately diagnose patients, provide evidence-based treatment recommendations, and write efficient encounter visit notes.
