Family Mentor

About the Role:

• The Family Mentor is an integral member of a patient’s multidisciplinary care team, ensuring caregivers successfully integrate into Equip treatment.
• This role focuses on proactively identifying barriers, personalizing programming, and building caregiver skills to achieve lasting recovery.

Responsibilities:

• Provide skills-based individual and group sessions to caregivers, implement motivational strategies, and monitor adherence to care plans.
• Conduct proactive outreach, missed appointment recovery, and problem-solving to ensure full engagement and personalization of care.
• Participate in multidisciplinary treatment team huddles and complete timely documentation of caregiver interactions.

Qualifications:

• 2+ years of experience working with families in a healthcare setting, including coaching, case management, or social work.
• Lived experience as an eating disorder support for a loved one in stable recovery.
• Strong communication, organizational skills, and comfort working remotely and with new technologies.

Compensation & Benefits:

• Earning potential range of $54,000 to $65,000 base pay plus 15% annual performance-based bonus.
