Bilingual Spanish Care Coordinator

Role Summary:

• This is a high-volume, phone-based position supporting transitions-of-care for members after hospital and skilled nursing facility discharges.
• You will conduct outbound telephonic outreach, schedule follow-up care, and coordinate identified needs.

Responsibilities:

• Conduct high-volume outbound calls to members and complete time-sensitive outreach protocols.
• Engage members to schedule appointments, arrange referrals, authorizations, DME, medication refills, and document outcomes.
• Work as a team with clinical and care management staff and escalate clinical concerns appropriately.

Requirements:

• Minimum 1 year healthcare experience and completion of an accredited Medical Assistant program.
• Bilingual English/Spanish required and proficiency with Microsoft Outlook, Excel, and Word.
• Able to spend most of the workday on the phone and follow a Pacific Time schedule.
