Job Summary and Responsibilities:

  • Provide critical leadership in advancing high-quality, patient-centered care for the Central Area Region.
  • Oversee patient flow, discharge planning, and readmission prevention across multiple hospitals.
  • Ensure alignment with systemwide standards and regulatory requirements.

Strategic Leadership and Operational Excellence:

  • Provide strategic and operational leadership for Care Management functions across all hospitals in the region.
  • Lead implementation and standardization of system policies, procedures, and tools.
  • Monitor and report regional performance outcomes, addressing variances with local and system leaders.

Collaborative Partnerships and Stakeholder Engagement:

  • Build and sustain strategic partnerships with system, regional, market, and hospital leaders.
  • Serve as primary Care Management liaison for the Region Chief Nursing Officer.
  • Collaborate with post-acute and community partners to ensure seamless patient transitions.

Qualifications:

  • Master's degree and Bachelor's in Nursing required.
  • Minimum of 10 years in acute care management including 5 years in a leadership role overseeing multiple facilities.
  • Registered Nurse license and Accredited Case Manager certification preferred.
