About the Role:
- Directly support provider practices in New Jersey to implement products and execute ACO initiatives.
- Accountable for wellness visits, transitions of care, and active management to meet practice goals.
Qualifications:
- Must be a New Jersey resident with a college degree or 4-6 years of medical practice management experience.
- 3+ years EHR experience; strong communication and project management skills required.
