Care Coordinator

Responsibilities:

• Work collaboratively with a multi-disciplinary team to ensure a high-quality patient experience.
• Ensure processes are followed for efficient, effective, and timely care delivery.
• Identify potential quality issues and escalate appropriately.

Qualifications:

• 1+ year experience in a medical practice with medical terminology knowledge.
• Willingness to work weekends and holidays.
• Excellent verbal and written communication skills.
