Referral Coordination:
- Manage and analyze referral responses, including acceptance, non-admissions, and pending patients.
- Serve as the lead coordinator of all referrals and maintain accountability for all aspects associated with the response to eReferrals.
- Maximize efficiencies with vendors and referral partners to ensure a streamlined intake process.
Communication and Collaboration:
- Communicate regularly with referral sources, including doctors’ offices and hospitals, to provide updates.
- Work collaboratively with internal teams and external partners.
Continuous Improvement:
- Participate in meetings, committees, and special projects to continuously improve processes and outcomes.
- Maintain compliance with all company policies, procedures, and applicable federal and state regulations.
