The Oncology Nurse Navigator (ONN) is a professional RN who provides personalized support, guidance, advocacy, and education to Members and their caregivers affected by cancer. They possess oncology-specific clinical knowledge required to guide Members, families, and caregivers to assist overcoming healthcare system barriers. Using the nursing process, they provide education and resources to facilitate and support informed decision-making and timely access to quality health.
A Triage Nurse is a professional registered nurse with oncology-specific clinical knowledge that offers individualized care and clinical guidance to patients, families, and caregivers to assist with ongoing healthcare needs. Provides care to patients by triaging oncology patient calls regarding treatment, surgery, and appointment information and monitors and provides patient symptom management.
Provide and facilitate amazing patient-centered clinical care to our patients. This position involves working three 12-hour shifts per week. Key responsibilities include ongoing clinical and psychosocial assessment of new patients, identifying and addressing barriers to care, and reducing care gaps by frequent engagements with patients. The role also involves assessing the need for and educating patients on equipment, timely response to abnormal results, and proactively triaging patients for emergency services.
