As the APC, Centralized Follow-Up (CFU) you will spend most of your time on the computer in a virtual environment providing care to diagnose and treat issues and conditions related to reproductive, sexual health, abortion, and gender-affirming care. This person will also spend their time using the Electronic Health Record system EPIC, reviewing labs and diagnostic imaging, triaging patient’s questions, handling prescription requests, documenting patient interactions, and running reports.

Identifies opportunities in the medical home and manages and coordinates clinical care of defined populations with identified preventive care and/or chronic disease needs in the outpatient setting. Functions as a resource to patients and families and actively role models patient and family centered care. Contributes to the development and maintenance of a patient/family centered care delivery system and promotes effective resource utilization.
