About the Role:
- Support the connection between patients, families, and the compassionate care they need.
- Manage referrals, coordinate information, and ensure patients are connected with appropriate services in a timely manner.
- Partner with clinical teams and referral sources to create a seamless experience.
Qualifications:
- High school diploma or GED required; Associate's degree preferred.
- Minimum 1 year of experience in a healthcare setting.
- Knowledge of medical terminology and proficiency in Microsoft Office Suite.
