The Care Coordinator ensures linkages for CCBHC patients and the community across the health services spectrum, including physical and behavioral health. They work collaboratively with staff, providers, and community resources to assist clients in overcoming barriers to care and improving health outcomes. The role requires strong communication and problem-solving skills.

Be accountable for improving clinical outcomes for empaneled pediatric patients by overseeing their medical care. Participate in daily clinical huddles, review complex patient cases, and engage directly in patient care as a Pomelo provider. Monitor adverse events and lead development of evidence-based medical protocols related to pediatric care. Actively participate in on-call schedules.
