The Part Time LPN/LVN Care Coordinator will work within our contact center, interfacing with patients, providers, and families in our palliative care program, managing day to day functions such as educating patients by phone and scheduling them for palliative appointments. You'll be working with clinical files to close out palliative visits and enter notes into the systems, spending part of the day on the phones and part of the day doing administrative work.
