The Clinical Intake Coordinator is an integral member of the ECM care management team, working directly with clients to understand their needs and connect them to the correct resource on our primary care management team. To be successful in this role, you will be personable and professional via phone, quickly review medical charts, provide person-centered care, and prioritize tasks effectively to support client engagement. You will also motivated to help high need individuals with complex care needs.

The Clinical Quality Specialist role exists to uphold and strengthen the quality of care within the organization. This position ensures that clients and clinicians are supported with the right resources, processes, and interventions to deliver excellent, evidence-based care. Seeking a BCBA with an active BCBA certification and licensure and 2-4 years of experience in ABA instruction. Must have strong knowledge of ABA techniques and graphing procedures and an ability to lead and instruct therapists.

The Clinical MDS Account Manager will be responsible for coordinating patient care, ensuring compliance with regulatory guidelines, and supporting reimbursement accuracy through effective utilization of managed care resources. This role requires strong clinical judgment, excellent communication skills, and the ability to collaborate across interdisciplinary teams.

Provides telephonic assessments of members’ medical, psychosocial, physical and spiritual needs. Develops, implements, and monitors person centered service plan, and ensures continuity of care across all healthcare settings. Collaborates with the interdisciplinary team, PCP, member/member’s representatives, and specialists. Utilizes benefit structure and community-based resources to meet member’s needs.
