Proactively grow the specialty pharmacy patient population by making outbound phone calls to eligible patients and enrolling them with Shields Partner Pharmacy. Educate patients on the services offered by the Shields Specialty Pharmacy Program. The person will communicate with patient over the phone and provide them the compelling reason why the Shields Partner Pharmacy program is superior to any other comprehensive pharmacy care model.

As a Patient Engagement Specialist, you are responsible for supporting both patients and clinical care team members with engagement efforts. Patient Engagement Specialists will play a critical role in ensuring that patients achieve initial activation and ongoing engagement with Evergreen Nephrology and their local care teams. The Patient Engagement Team will partner with our market operations teams across the US to ensure patients are actively engaging in their care, working with their interdisciplinary care team, and achieving their kidney care and overall care goals.

Serve as a crucial link between physicians and industry partners by managing and streamlining healthcare provider requests across various service areas, ensuring efficient delivery of resources and support to medical professionals. Coordinate and manage physician requests for samples, patient assistance programs, and educational resources. Serve as the primary point of contact for healthcare professionals, maintaining clear and professional communication. Manage and track request fulfillment to ensure timely delivery of services.

Collaborate with clinicians to develop personalized care plans, provide nutritional counseling, and act as an accountability partner. The role involves monitoring member progress, integrating technology into counseling, and recommending dietary supplements. This position is remote-based, with occasional in-person work at our San Mateo clinic.

Be an integral part of the close-knit team of Kairros Consultants who work with and support people who have sustained a psychological or physical injury in the workplace. As a valued team member, you will have the responsibility of assessing needs and then providing effective solutions and strategies to ensure that Kairros clients receive a safe return to work within their same employer or with a new employer as part of their rehabilitation and recovery.

This role ensures timely, coordinated, and high-quality care delivery to patients in their homes. You will collaborate with Patient Care Managers to process referrals, manage daily scheduling tasks, and provide operational support for the Albuquerque Home Health and Hospice Agency.

Acts as the direct contact for clients seeking advice and guidance through the “live chat” function within the myVCA mobile application. Responsible for providing an excellent experience to clients, making them feel valued and affirmed, and providing sound and relevant medical guidance. Also help with other administrative-related queries. The position is part-time and available to credentialed veterinary technicians who have the time and ability to engage with clients, virtually, from home.
