The CDCES will guide members throughout their healthcare journey, aiming to improve their diabetes self-management through virtual communication. The CDCES will educate and counsel members with out-of-control diabetes, working to stabilize their condition and graduate them from the specialty clinic. The CDCES will collaborate with a team of healthcare professionals to develop care plans and optimize medication regimens.
