This is a fully remote Clinical Care Manager position where you will use a proprietary software platform to optimize engagement with your patients. Nurses will work from home reviewing patient vitals, managing care plans, responding to alerts, and providing education. As a staff level telemedicine clinician in Keeps Medical Group (KMG), you will care for patients on our telemedicine platform, delivering quality medical expertise through virtual care, and collaborating with customer service for operational support, while improving the quality of care and operational processes.

Provides direct patient care for MAT, substance use and other psychiatric disorders both in-person and via telemedicine. Works closely with Eleanor Health Practice Administrator, Lead Clinician, and other team members to ensure seamless integration of medical services with whole-person support services. Ensures medical care and documentation are in compliance with all applicable state and federal laws.
